  • Alcona Health Centers is a network of healthcare clinics serving northeastern and northwestern lower Michigan in Alcona, Alpena, Iosco, and Emmet counties. 

    We have developed this website in effort to provide the public with useful healthcare-related information such as our mission statement, locations, hours of operations, and our healthcare providers.  In addition to this there is detailed self-help information, links to wellness sites, info about diseases, tests and  medications.  Our website truly offers a wealth of medically-related information!

  • Health Plan of Michigan
  • Health Plan of Michigan is the largest Medicaid HMO in the State of Michigan, providing health care to over 236,000 Medicaid enrollees through a contract with the Michigan Department of Community Health. 

    HPM has achieved "Excellent" Accreditation from the National Committee for Quality Assurance (NCQA). That is the highest level that a health plan can obtain. In addition, HPM was rated 14th among all Medicaid health plans in the country according to U.S. News and World Report magazine.

    Health Plan of Michigan's Disease Management Program goal is to help members manage their health. Members learn more about their condition and the risks; change the lifestyle choices that could make their condition worse; learn how to become better at taking care of themselves; and communicate better with their doctor. 

    Enrollee Services - Call HPM Member Services at 1-888-437-0606 any time you have a question about your health care. Our Member Services Specialists are courteous and patient. You can reach us during normal business hours Monday through Friday from 8:00 AM to 5:00 PM. If you have an emergency after hours, you can reach HPM 24 hours a day, 7 days a week. There is a doctor on call to answer your emergency questions.

    Health Plan of Michigan can arrange for an interpreter to speak to you in any language. Call Member Services at 1-888-437-0606 and we can help. If you are hearing impaired, you may call 1-800-649-3777. The Michigan Relay Center makes it possible for hearing-impaired and/or speech-impaired persons to call Health Plan of Michigan. They can be reached 24 hours a day, 7 days a week.

    This website has all the information you will need to understand your CareSource benefits. You can find a list of providers, Pharmacies and Community Resources. You can also find Member Handbooks and Health Surveys – all designed to keep you and your family healthy!

We will do all that we can to help you and your family with your health care needs. Our Michigan Medicaid plan offers no-cost comprehensive health coverage with no monthly premiums and no co-payments for any benefit.

    As a Molina member, you have access to thousands of physicians and hundreds of hospitals, as well as 24-hour Nurse Advice line and free transportation services .  

    Free Transportation is provided to covered services. Transportation is provided only when you have no other means to get to your doctor appointments, x-rays, lab tests, or other medical care.     • Alpena Regional Medical Center

      District Health Department #2 works to reduce the incidence of communicable diseases and infection through data collection, education, treatment, referral for care, reporting of illness and enforcement of laws.

      Routine communicable disease activities and services are provided during regular health department hours. Staff is available during off hours for emergency situations.
