If you do not have insurance, we have SLIDING FEE available to qualified individuals!  This plan can significantly reduce the cost of your office visits.  Just call our office to inquire! 

Payment is expected at the time of service for all cash paying patients.  If you have insurance and are expected to pay a co-pay and/or deductible, that amount is due on the day of service.     

We Participate with Medicare, Medicaid, Medicaid HMO's such as Community Choice of Michigan,  All Blue Cross and Blue Cross of Michigan Plans, Northern and Northeast Health Plans, Tricare, PPOM, Blue Care Network, and Priority Health.  

We will courtesy bill for all commercial insurances. 

For all insurances, a valid Medicaid card must be presented at each visit.
