Our Alcona Health Centers Mission Statement: The purpose of Alcona Citizens for Health, Inc., shall be to promote, develop and operate such facilities and programs as are necessary to provide quality health care to all residents of Alcona County and the surrounding areas, regardless of ability to pay.

Alcona Health Centers is a federally qualified healthcare center (FQHC).  We are a recognized Patient Centered Medical Home and are accredited by The Joint Commission.  Our network of healthcare, dental clinics and school-based clinics are situated in northeastern Michigan in Alpena, Alcona, and Iosco counties, and in northwestern Michigan in Emmet County. 

As a Patient Centered Medical Home, we provide comprehensive primary care for children, youth, and adults in a setting that facilitates partnerships between the patient, their personal healthcare provider, and when appropriate; the patient’s family.

We offer a wide variety of services; family medicine, osteopathic medicine, internal medicine, obstetrics, behavioral health, dental health, and school-based healthcare services.  We offer health education to help you learn how to better manage your health.  Our case-management team further assists to coordinate your healthcare needs. 

Alcona Health Centers’ Hospitalists specialize in caring for patients in the hospital from admission to discharge.  All AHC patients who are admitted to Alpena Regional Medical Center will be under the direct care of one of our hospitalist physicians.  Our hospitalists also provide care to individuals who are not patients of Alcona Health Centers.  NOW YOU CAN RENEW YOUR PRESCRIPTIONS ONLINE!  Click on the link below to access our PORTAL OF GOOD HEALTH.  https://www.rxsystems.com/portalnet/default.asp?sn=18

All Alcona Health Centers’ patients may purchase a wide variety of over-the-counter (OTC) medications at a significant savings in our offices in Alpena, Harrisville, Harbor Springs, Lincoln, Oscoda, and Ossineke.  You may qualify for reduced-cost prescription medications through our clinic’s 340-B Program.  It is available to our patients who qualify for 0% Sliding Fee.  Ask any of our staff how you can learn if you qualify for this program.  <?xml:namespace prefix = o />

We also assist patients in obtaining free-to-low-cost medications from participating pharmaceutical companies through our Patient In Need Program.  Ask any of our staff on how to apply for this program. 

/Alcona Health Centers is accredited by the Joint Commission.  To maintain and earn accreditation, organizations must have an extensive on-site review by a team of Joint Commission health care professionals, at least once every three years.  If you have a complaint about a Joint Commission-accredited health care organization, you are encouraged to first bring your complaint to the attention of the health care organization’s leaders.  If this does not lead to a resolution, you may bring your complaint to the Joint Commission for review.  Submit your complaint in writing to Office of Quality Monitoring.   The Joint Commission   1 Renaissance Blvd.  Oakbrook Terrace, IL  60181; online at complaint@jointcommission.org <?xml:namespace prefix = o <?xml:namespace prefix = o /> or by fax (630) 792-5636.  Summarize the issues in one or two pages and include the name, street address, city, and state of the health care organization.  You may either provide your name and contact information or remain anonymous.  It may be necessary to share the complaint with the subject organization in the course of the complaint investigation. 

As a Patient Centered Medical Home, we provide primary care that is care comprehensive primary care for children, youth and adults.  The PCMH is a health care setting that facilitates partnerships between individual patients, and their personal healthcare provider, and when appropriate, the patient’s family.

Each patient has an ongoing relationship with a personal physician and/or healthcare provider that is trained to provide first contact, continuous and comprehensive care.  Your medical care is physician-directed.  – Your personal physician leads a team of individuals at the practice level who collectively take responsibility for your ongoing medical care. 

You will notice our whole person orientation; your personal physician is responsible for providing for all the patient’s health care needs or taking responsibility for appropriately arranging care with other qualified professionals. This includes care for all stages of life; acute care; chronic care; preventive services; and end of life care.

Care is coordinated and/or integrated across all elements of the complex health care system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient’s community (e.g., family, public and private community-based services). Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

Quality and safety are hallmarks of the medical home:

·         Practices advocate for their patients to support the attainment of optimal, patient-centered outcomes that are defined by a care planning process driven by a compassionate, robust partnership between physicians, patients, and the patient’s family.

·         Evidence-based medicine and clinical decision-support tools guide decision making

·         Physicians in the practice accept accountability for continuous quality improvement through voluntary engagement in performance measurement and improvement.

·         Patients actively participate in decision-making and feedback is sought to ensure patients’ expectations are being met

·         Information technology is utilized appropriately to support optimal patient care, performance measurement, patient education, and enhanced communication trained to provide first contact, continuous and comprehensive care.

·         Your medical care is physician-directed.  – Your personal physician leads a team of individuals at the practice level who collectively take responsibility for your ongoing medical care.

·         Practices go through a voluntary recognition process by an appropriate non-governmental entity to demonstrate that they have the capabilities to provide patient centered services consistent with the medical home model.
